Microscopic colitis was recognized in 10% of all sufferers with nonbloody diarrhoea referred for colonoscopy and in virtually 20% of these older than 70 years. Microscopic colitis includes two distinct histological diagnoses: collagenous colitis and lymphocytic colitis. In collagenous colitis, a thickening of the subepithelial collagen layer is seen, together with a continual mononuclear irritation in the lamina propria. Lymphocytic colitis is characterised by an elevated number of intraepithelial lymphocytes, but the collagen layer is regular. Treatment of microscopic colitis was up to now based on observational research and anecdotal proof. The fluctuating clinical course of microscopic colitis, characterised by spontaneous remission intervals might result in bias in uncontrolled research.
